The following forms are also available in the Clubhouse hallway display board. All forms are to be returned to the Summerfield Civic Association Office.  We are open Monday through Friday from 8 AM – 12 Noon, however there is a mail drop slot located under the office window for your convenience. There is also an EXTERIOR mail slot for the Clubhouse, located to the far right of the front doors with a gold flap.
