The following forms are also available in the Clubhouse (located in the stack trays near the copy machine). 
All forms are to be returned to the Summerfield Civic Association Office.  We are open Monday through Friday from 8 AM – 12 Noon, however there is a mail drop slot located under the front window for your convenience.

Please note that the Architectural Change Request Form and the Landscape Change Request Form need to be submitted in duplicate (2 copies).
